'Man or mouse' Cameron challenged over Heathrow

A senior Conservative MP challenged David Cameron to decide if he is "a man or a mouse" over calls to expand Heathrow airport on Tuesday.

Former environment minister Tim Yeo, writing in The Daily Telegraph, asked if Cameron would preside "over a dignified slide towards insignificance" by Britain or if he possessed "a trace of Thatcher, determined to reverse the direction of our ship".

The call to expand Heathrow is largely driven by economic concerns. Housing minister Grant Shapps said last week that "all options" should be considered to ensure Britain remains a "great trading nation".

Conservative Party policy is against expansion, however.

Yeo, previously an opponent of expansion due to environmental concerns, said those fears were becoming groundless in the face of European Union carbon emissions caps that he said would force airlines to use cleaner planes to land at the airport.

"The environmental objections are disappearing," he said.

"Last January, greenhouse gas emissions from flying were brought within the EU cap. Indeed, we could cover the whole of Surrey with runways and not increase emissions by a single kilogram: if Heathrow expands, so remaining the European destination of choice, airlines will fly their newest and quietest aircraft to it."

Transport Secretary Justine Greening said there was cross-party consensus against a third runway.

"What we do need to do is to start the process of saying well, if we're not going to have a third runway, how do we make sure we have got the hub capacity that our country needs going forward?" she told BBC Radio 4's Today programme.

She added that a possible new hub airport was "clearly one of the options" to be considered instead.

Those simple but often overlooked steps can help protect some of the most critically ill patients ? those on ventilators ? from developing deadly pneumonia. And if they knew about them, family members could ensure the steps weren't forgotten.

Hospitals are rife with infections and opportunities for medical mistakes. Now, a nearly $9 million project at Johns Hopkins University aims to combine engineering with the power of patients and their families to prevent some of the most common threats.

The idea: Design patient safety to be more like a car's dashboard, which automatically signals drivers when the oil needs changing or if a passenger forgot to buckle up, or like the countdown systems that make sure no step is missed when a satellite is launched.

Today, safe, quality care largely depends on individual health workers remembering hundreds of steps without good ways to tell if they forget one, said Hopkins' patient safety expert Dr. Peter Pronovost. Getting it right takes what he calls "almost heroic efforts."

And too often, the people best able to spot early warning signs ? patients and their families ? are treated as passive bystanders rather than encouraged to participate in their care, he said.

"Who knows better than the family?" asked Dr. George Bo-Linn of the Gordon and Betty Moore Foundation's new Patient Care Program.

The foundation, created by Intel co-founder Gordon Moore, announced Tuesday that it is funding the Hopkins work as the first step in a planned 10-year, $500 million effort to improve patient safety and family engagement in hospitals around the country. Separately, the Institute of Medicine has signed on to help, partnering with the National Academy of Engineering, to bring together top experts on how to design safety systems.

Sometimes the failure to merely discuss treatment with patients or their families causes the harm. Consider Nicole D. James, a commercial property manager in Elkridge, Md., who undergoes frequent overnight hospital stays to treat the intense pain of sickle cell anemia. One such visit turned into a miserable two-week stay when the ER doctor added a powerful antibiotic to James' usual pain treatment without telling her first ? a drug that worsened her sickle cell crisis.

It turned out the doctor had spotted a shadow on James' lung X-ray that he thought was pneumonia but that she could have told him, and her regular physician confirmed, really was old scar tissue.

"I know what's normal for me," said James, 37, who now insists that relatives drive past closer hospitals for Hopkins, where her doctor practices, no matter how late at night or intense her pain. Because the doctors know her, "I am not just somebody lying on the table. I am part of the team."

Tens of thousands of preventable deaths occur in U.S. hospitals every year. Numerous programs are under way to improve patient safety. Among them is the government's Partnership for Patients, funded by $1 billion from the new health care law, that is helping hospitals adopt proven safety strategies.

Hopkins' Pronovost led the creation of one of the most well-known ? a simple checklist that ensures hospital workers follow steps that lower the risk of deadly bloodstream infections from common IV catheters. That checklist now is being used in ICUs nationwide, and the government reported last year that those infections have plummeted by 60 percent as a result.

But catheter infections are just one of a dozen serious hospital-caused harms that threaten ICU patients, Pronovost said. Rather than fighting them one at a time, his new project will target multiple ICU threats simultaneously ? from ventilator-associated pneumonia to deadly blood clots ? without relying on old-fashioned paper checklists and with more family involvement.

It will require linking medical devices that today don't talk to each other, he said. For instance, pumps that deliver narcotic painkillers aren't linked to other devices that monitor breathing. If connected, the painkillers could be stopped automatically at the first sign of respiratory problems, a known side effect.

Other protections are far less complex. Keeping the head of the bed elevated at least 30 degrees and good oral hygiene are among the steps that help fend off the pneumonia that kills 36,000 people a year while they're on ventilators. That's something family members can watch for ? or they even can be trained to brush a loved one's teeth around the breathing tube.

Already, Hopkins has introduced a "family involvement menu" of care items. "We believe that you know the person that we are caring for far better than we do," the menu reads.

Ultimately, Pronovost envisions an iPad-like device that allows both health workers and family members to see at a glance which of dozens of required daily care steps have been performed and which still need to be.

"The beauty of checklists is it gives you that constant visual reminder," and an electronic, automated version is the next logical evolution, said Ann Marie Pettis, director of infection prevention at the University of Rochester Medical Center, who called the project intriguing.

Engaging the patient and family in some ways is tougher. Hospitals have had a hard time encouraging people to ask doctors and nurses if they've washed their hands, said Gina Pugliese of the Premier Safety Institute, a hospital improvement alliance.

"So many patients and families are hesitant to ever question the doctor," she said. "It's so important they get involved."

But Mr. Boehner never used the word ?hope.??Here is what he did say, and the context for it.?

At the lunch session, Stephanie Kirchgaessner of the?Financial Times noted that the latest NBC News/Wall Street Journal?poll had Mitt Romney receiving zero percent support from?African-American voters. ?

?Can the Republican Party continue to win?general presidential elections if they don?t appeal to more voters?than they are today in terms of nonwhite voters,? she asked.

Boehner replied:??You know we?ve never done well with those groups. But think about who?this economic downturn has affected the most: blacks, Hispanics, young?people. Fifty percent of college graduates are unemployed or?underemployed. And I think our economic message in this election cycle?will help us recruit more of those groups than we would have?otherwise.??

He continued: ?But I think it?s important for our party,?if we?re going to be a national party, we?ve got to reach out. And?that means showing up in their neighborhoods. It?s a tall order, but?it can be done."

Ms. Kirchgaessner followed up by asking, ?Has it happened so far??Is your party doing that right now??

Certainly, we are very disappointed by the verdict at the US District Court for the Northern District of California (NDCA), and it is regrettable that the verdict has caused concern amongst our employees, as well as our loyal customers.

However, the judge?s final ruling remains, along with a number of other procedures. We will continue to do our utmost until our arguments have been accepted.

The NDCA verdict starkly contrasts decisions made by courts in a number of other countries, such as the United Kingdom, the Netherlands, Germany, and Korea, which have previously ruled that we did not copy Apple?s designs. These courts also recognized our arguments concerning our standards patents.

Ryan Lochte is moving on.

No, no, no ... not from swimming, but from Gainesville, Fla. The former Florida Gator has decided it's time to move out of his college town.

"Being in a college town for so long, it does get old," Lochte said.

He's got tons of support in Gainesville ? coaching, nutritionist, strength coach and the like. But it's apparently not enough to keep him in "Gator Country."

"I'm getting older and I think it's time for me to move on," Lochte said. "I'm still going to a little training there, but I definitely want to move somewhere else and probably start a new life."

Lochte is planning to move to California, probably the Los Angeles area. He needs to stay warm ? and needs to keep surfing, he said.

Everyone has an opinion, and if they all stink, then Pew must have one hell of a tolerance for foul odors. The research group recently surveyed a number of mobile phone owners to determine their primary complaints, and while it's not much of a surprise, slow network performance stole the show. In all, 77 percent aired at least some dissatisfaction with download speeds, and nearly half of all respondents cited frequent frustration. The story is similar for dropped calls, as 72 percent of those surveyed claim to experience the annoyance at least occasionally. While less widespread, the distribution of those who receive unwanted marketing attempts via either telephone calls or text messages is roughly equal -- it seems reasonable to assume that many respondents are harassed by both means. Sorkin stands by "Newsroom", denies firing writing staff

LOS ANGELES (Reuters) - Oscar-winning writer Aaron Sorkin on Wednesday vigorously defended his controversial show set in a fictional cable TV newsroom, and refuted reports that he fired his writing staff.

Sorkin said that "Newsroom," which mixes real news stories with fictional characters in a workplace drama, was not intended to thumb its nose at the way established television channels have covered big news events.

Sorkin, creator of the idealistic White House drama "The West Wing" and the award-winning screenplay writer of Facebook movie "The Social Network," also told journalists that the next episode of "Newsroom" will deal with the capture and killing by U.S. forces in 2011 of Osama bin Laden.

"Newsroom" has been one of the most talked about new shows on U.S. television since its debut on HBO in June, with critics and fans divided over its utopian portrait of what TV news could be and its portrayal of news reporters.

"I have only ever tried to write the way I write," Sorkin told the Television Critics Association. "I haven't tried to figure out what most people will like, and then give it to them. I try to write what I like, and what I think my friends will like, and then I keep my fingers crossed that enough other people will like it that I can keep doing it."

"Anytime people are talking this much about a TV show is good for television, good for people who watch and good for people who work in television," he said.

Sorkin took the opportunity to deny a media report last month that claimed he had fired the writing staff for the show's second season, which has already been ordered by HBO.

"The writing staff was not fired. ... Seeing that in print has scared the hell out of the writers. They are coming to work early and being polite to me. I love the writing staff. I think they are a fantastic group," he said.

Sorkin's use of recent news events, including the 2011 BP oil spill in the Gulf of Mexico, the Japanese tsunami and nuclear reactor meltdown, and the shooting of U.S. Representative Gabby Giffords, has offended many journalists because he appears to be critical of reporting and errors made while reporting breaking news.

"I didn't want to make up fake news. I didn't feel we would be able to relate to fake news," Sorkin said. But he added, "I didn't do it so I could leverage hindsight, so I could make our characters smarter."

Sorkin also shrugged off criticism that his female characters mess up more on their jobs than the male journalists, saying all the characters in the show were deeply flawed.

And he said the first season has already been filmed, so he could not make changes even if he wanted to.

Jeff Daniels plays cranky TV anchorman Will McAvoy, who is on a mission to present the news without commercial or corporate obstacles. Daniels has made clear he had no time for critics.

"It took me a long time as an actor to stop reading you," Daniels told the TV critics on Wednesday. "If Aaron is happy, if HBO is happy, and I am happy when I leave the set, I hate to say it is great. ... There is nothing you can tell me, I am sorry to say, that will help me as an actor."

Uneasy partners Paes and Mirza shrug off row

Leander Paes and Sania Mirza insisted the controversy surrounding their Olympics mixed doubles partnership hasn't damaged their medal hopes.

The Indian duo defeated Serbia's Nenad Zimonjic and Ana Ivanovic 6-2, 6-4 in the first round at Wimbledon on Thursday, their first outing since an ugly selection wrangle split the Olympic squad.

The pair are playing together in acrimonious circumstances after Paes demanded an assurance that only he would be paired with Mirza, rather than Mahesh Bhupathi or Rohan Bopanna, following the row over Bhupathi's refusal to play with Paes in the men's doubles.

Mirza previously admitted she was uncomfortable at being used as "bait" to keep Paes happy after winning two Grand Slam titles with Bhupathi.

But, despite the stormy backdrop to their partnership, Mirza claimed their relationship is beginning to flourish on court.

"The secret of success in doubles and mixed doubles is communication. (It's working well) with him playing at the back and me playing at the net," she said.

Paes agreed that they are starting to build a rapport.

"We play as individuals but I try to encourage her all the time and she is getting better and better," he said.

With all the Indian men's and women's doubles teams eliminated from the Games, Paes and Mirza are their country's last hope of a medal and they remain in the hunt after a comfortable first round win on Court 14 at Wimbledon.

Next up for Paes and Mirza is a quarter-final clash against Belarusian top seeds Victoria Azarenka and Max Mirnyi.

Even Mild Depression, Anxiety Hurts the Heart: Study

TUESDAY, July 31 (HealthDay News) -- Even mild depression or anxiety may raise your risk of dying from cardiovascular disease and other causes, according to British researchers.

And the greater the level of psychological distress, the higher the odds of death from heart disease, the researchers say.

"The fact that an increased risk of mortality was evident, even at low levels of psychological distress, should prompt research into whether treatment of these very common, minor symptoms can reduce this increased risk of death," said lead researcher Tom Russ, a clinical research fellow at the Alzheimer Scotland Dementia Research Center of the University of Edinburgh.

For the study, published online July 31 in BMJ, Russ and colleagues analyzed 10 studies of men and women enrolled in the Health Survey for England from 1994 to 2004. Data on more than 68,000 adults aged 35 and older was included overall.

Each study looked for connections between chronic psychological distress and the risk of dying from heart disease and other causes, including cancer.

Pooling data in this way is called a meta-analysis. In such a study, researchers look for common patterns across several studies.

Over eight years' follow-up, the researchers found even very mild depression or anxiety -- subclinical levels -- raised the risk of all-cause death, including cardiovascular disease, by 20 percent. Looking specifically at death from heart disease, mild psychological distress raised this risk 29 percent, the study found.

For the highest level of depression or anxiety, the risk of all-cause death rose 94 percent, the researchers found.

Risk of death from cancer was increased 9 percent in cases of very severe depression or anxiety, the investigators found. Lower levels of psychological distress were not associated with increased risk of cancer death.

An individual's actual risk of death remains small, however, and people shouldn't assume they are doomed to an early death if they suffer from a psychological disorder.

Dr. Glyn Lewis, a professor of psychiatric epidemiology at the University of Bristol in England and author of an accompanying journal editorial, said evidence linking stress to heart disease continues to mount.

"If we can reduce the psychological impact, then this should reduce the biological response," he said. But how to accomplish that remains a puzzle.

A type of psychological treatment called cognitive behavioral therapy is designed to help people change the way they respond to potentially stressful events, Lewis said. Cognitive behavioral therapy teaches patients to change their thinking about situations and to react less emotionally.

"This might help people with [clinical] depression, but there is no evidence that this might help the much larger numbers of people who have low-level symptoms that are below the diagnostic threshold for depression," he said.

While antidepressants might improve depression, previous studies have linked their use to greater risk of heart disease, according to background research in the study. About 7.5 percent of United Kingdom residents have depression and anxiety disorders, Lewis said.

Changing this stress-disease dynamic might also involve keeping common risk factors for cardiovascular disease in check, another expert said.

Dr. Gregg Fonarow, a professor of cardiovascular medicine at the University of California, Los Angeles, said many studies have demonstrated an association between depression and anxiety and cardiovascular events, cardiovascular deaths and all-cause mortality.

But so far, no evidence has shown that treating depression or anxiety reduces the risk of heart disease, Fonarow said.

Many different mechanisms may connect psychological distress to cardiovascular disease, including increased sympathetic nervous system activity, stress hormones such as cortisol, chronic inflammation, unhealthy lifestyle factors and inattention to early symptoms, he said.

"For people with depression or anxiety, focusing on proven cardiovascular risk factor interventions, including maintaining healthy blood pressure, body weight, cholesterol levels, engaging in regular exercise and not smoking, may represent the best course of action to lower their cardiovascular risk," he advised.
